To reorder contacts go to the home page click on contact us. Enter patients name, address and phone number plus the quantity of contacts you wish to reorder.
If you wish to pay for them on line and have them shipped to you please include your credit card number, name on the card and expiration date. There is a $2.00 processing fee for online transactions. There is a shipping fee which may vary $1.50 to $6.00 depending on size of order.
